Use this checklist to make sure you're staying healthy and at home rather than back in the hospital.
▢ Use written instructions from your doctor for post-hospitalization care. If you have written instructions, follow them! If not, call your admitting doctor or primary care doctor and ask what they recommend. Ask for them in writing or get a friend or family member to help you take notes.
▢ Schedule a follow-up appointment with your specialist or primary care doctor. It's very important to see your doctor within one to two weeks of returning home. They’ll help monitor your condition and keep your recovery on track.
▢ Remind yourself to take your medications. Tip: Set an alarm on your phone or enlist the help of a friend or family member to prompt you.
▢ Make sure you understand your medications and what they do. Knowing how your medications help your condition, and how they may interact with each other (and with foods), will help you keep your blood sugar balanced.
▢ Know your “red flags.” Do you know how to tell if your condition is worsening? Make sure you know the signs and when to call the doctor or nurse line.
▢ Use the buddy system. Ask a friend or family member to double-check your medications and doctor’s notes—and to make sure you’re following them correctly!
